How do I get my child evaluated?
If you plan on using insurance to pay for services you will need a referral from your child's pediatrician for an initial evaluation. Once we receive the referral we will contact you to schedule an appointment. Prior to the evaluation you will need to complete our intake forms. These forms provide us with information on your child's development and current level of function as well as outline some of our company policies. These forms will be sent to you when we schedule your child's evaluation. If you choose to use ESA or pay cash, we do not need a referral from your child's pediatrician, however, any medical notes would be appreciated. Please call for more information.
Will my insurance cover services?
We currently accept the following commercial insurance plans; Blue Cross Blue Shield (BCBS), UnitedHealthcare (UHC), and Cigna. We accept Health Choice Arizona and Arizona Complete Health through AHCCCS. We are also a vendor for ESA. Please check back for updates.
